There are several modes for CA supported by 3GPP, with one or two PA's and receivers.
Please, have a look to the attached paper (page 41).

Measurement gaps are there to let UE listens to other freq. defined in SIB
if you have two bands already configured in PC, SC why is a need to scan other?

UE measures the CQI for both the frequency bands while measurement starts for CA configuration: UE reports RSRP only for other layer once lets say on band 3, If better than configured threshold then UE configures CA but CA activation is only done once there is some data to be sent which can't be finished by first carrier only. During CA active mode UE reports CQI, PMI , RI for both layers to primary cell only. It's just like measurement gap you can measure other frequencies too.
