i've faced once and the problem was in the UE functionality...

From 3GPP, "If the radio interface Security Control Procedure fails, SECURITY MODE REJECT shall be sent to CN with a cause value indicating failure in the radio interface procedure".

Possibly the UE, or the security command message was corrupted. Is the failure repeatable?

Happened to me with NSN equipment + BB UE.
Root cause was in MSS. Definitive Fix is still pending.

try chnaging the UE and if the problem still persists then get the profile of ur USIM checked from MSS.
