This may have changed after the Great Financial Crises.

Previously, an employer in US (say //E) contact an agent to look for contractors. The agent then has to advertise the contractor position in local papers and has to prove to some US authority that there are no suitable/adequate candidates inside US. The agent then goes & recruit foreigners and sponsor the application of H1B visa for the foreigner.
